Patient Forms
We ask new patients to complete several forms - thank you for your help with this matter! We are also happy to email or text these documents to you - just let us know!
*The first two documents are information release forms that allow us access to any protected health information documents you wish us to see. This is important information which allows us to provide the best possible care to your child. **Please complete both forms for each child or young adult entering the practice.
*Please complete one "New Patient Demographics" form per family.
*Should you wish to send alternate caregivers (relatives, babysitters, friends) with your minor child to a visit now or in the future, we require a "proxy consent form" be on file
*Please share all applicable guardianship paperwork with our office upon establishing care, and with any change in guardianship status in the future.
